So, how do we do it?  One of our techniques we use with our clients is called Solution-Focused Brief Therapy (SFBT).  SFBT is a short-term process that helps people change by constructing solutions rather than dwelling on problems.  This technique helps the client identify elements of the desired solution, learns to build on these elements, and  forms the basis for ongoing change.  Rather than searching for the causes of the problem, the focus is on defining the changes and making them a reality: client wants his or her life to be different and what it will take to make it happen.
